I've uploaded a new patch which adds a MSCHAPv2 format:

I put this together a while back, but must have forgotten to post it.
I've updated it to John 1.7.6 w/ Jumbo-5 and everything seems to be in
working order. FWIW, I've been using this format during assessments of
PEAP-based wireless networks. Specifically, I run a fake AP which logs
the authentication attempts: http://www.foofus.net/?page_id=115
